Bioidentical Hormone Therapy: FDA-Approved vs. Compounded

"Bioidentical" describes a molecule, not a pharmacy. Several of the most-studied menopause hormones are bioidentical and come in an ordinary, FDA-approved blister pack.
The short answer

Bioidentical hormone therapy uses hormones molecularly identical to the ones your ovaries made, and several are FDA-approved: estradiol patches, gels, sprays and pills, plus oral micronized progesterone. Custom-compounded versions use the same molecules but aren't tested for dose, purity or safety. Major medical groups recommend the approved versions first.

I started looking into this after the third ad in a week promised me "natural, bioidentical hormones, custom-made for your body." A friend had just paid out of pocket for pellets at a wellness clinic, and she was convinced she'd dodged the risks of "regular HRT." I wanted to know whether she was right, so I went and read what the medical bodies actually say.

What I found is that the word "bioidentical" is doing an enormous amount of marketing work. It's a real scientific term — but it's been quietly fused with a second, very different idea: custom compounding. Pull those two apart and the whole topic gets much clearer.

The honest framing up front: this isn't an argument against bioidentical hormones. Some of the best-studied menopause treatments are bioidentical. It's an argument for knowing which bioidentical product you're being offered, because the approved and compounded versions are held to completely different standards.

What does "bioidentical" actually mean?

"Bioidentical" means the hormone has exactly the same molecular structure as the one your body makes: estradiol, progesterone, or testosterone. It describes the molecule only. It says nothing about who made the product, how it was tested, or whether the dose inside matches the label, and it isn't a regulatory category the FDA uses.

The contrast is with older, non-identical hormones. The Women's Health Initiative, the huge study behind most of today's HRT fears, used conjugated equine estrogens plus medroxyprogesterone acetate — a synthetic progestin. Neither is bioidentical. That history is a big part of why "bioidentical" sounds safer, even though the word itself doesn't promise safety.

What's the difference between FDA-approved and compounded bioidentical hormones?

Both can contain the same bioidentical molecules. The difference is oversight. FDA-approved products are manufactured to a fixed dose, tested in clinical trials, and sold with a full label listing risks. Compounded products are mixed individually by a pharmacy, usually to a prescriber's custom formula, and skip all of that testing entirely.

That's the whole distinction in one table. The molecule can be identical; the reliability of what's in the jar is not.

Which bioidentical hormones are already FDA-approved?

More than most people realize. Estradiol, the main estrogen your ovaries made, is FDA-approved as skin patches, gels, a spray, oral tablets, and vaginal products. Oral micronized progesterone, sold as Prometrium and generics, is FDA-approved too. A combined estradiol-and-progesterone capsule, Bijuva, was approved in 2018.

In other words, a woman who wants bioidentical hormones can get them at an ordinary pharmacy, often as inexpensive generics, with a known dose in every patch or capsule. The FDA's own consumer guidance makes exactly this point: it approves hormones identical to the ones women make in their reproductive years, and it recommends those over compounded versions.

What does the evidence say about compounded hormones?

Three independent groups reviewed it and reached the same conclusion. The National Academies of Sciences, Engineering, and Medicine found insufficient evidence of safety or effectiveness in 2020. The Menopause Society's 2022 position statement says compounded therapy isn't recommended. ACOG's 2023 consensus says it shouldn't be prescribed routinely when approved versions exist.

That knowledge gap is the heart of the problem. Most women using compounded hormones aren't choosing an unregulated product on purpose. They're responding to "natural" and "custom" in the marketing, and assuming the same safety checks apply that apply to anything else a pharmacy hands them.

Do bioidentical hormones help you sleep better?

They can, and the good news is that the helpful ones are already FDA-approved. Estradiol calms the hot flashes and night sweats that break sleep apart. Oral micronized progesterone taken at bedtime has modest direct sleep evidence, because the liver converts it into allopregnanolone, a calming compound that acts on the same receptors as sedatives.

Here's the detail that matters for compounded products: that sedating effect is tied to oral progesterone, which passes through the liver first. What are the specific risks of compounded hormones?

The biggest risk is simply not knowing your dose. Reviews have documented both underdosing and overdosing in compounded products. For a woman with a uterus, too little progesterone alongside estrogen means the uterine lining may not be protected, which raises the risk of endometrial hyperplasia, a thickening that can progress toward cancer.

  • Inconsistent dose: strength can vary between batches and even within a single jar of cream
  • Weak uterine protection: progesterone creams may not reach levels that protect the lining when you're also taking estrogen
  • No risk label: compounded products usually lack the warnings that come with approved hormone therapy, which can make them look safer than they are
  • Pellets: implanted under the skin for months, impossible to remove if levels run too high, and linked to supraphysiologic hormone levels

If you're using a compounded estrogen and still have a uterus, any unexpected bleeding after menopause needs to be checked by a doctor promptly. Don't assume it's a normal side effect of your hormones.

Can saliva or blood tests find your perfect hormone dose?

This is one of the most common sales points, and it doesn't hold up. The Menopause Society's 2022 position statement says saliva and urine hormone testing are unreliable for setting a dose. Hormone levels swing widely through the day, and a single number doesn't track how you actually feel or sleep.

In standard practice, dosing follows your symptoms. Your clinician starts with the lowest dose likely to help, checks in on hot flashes, sleep, and side effects after a few weeks, and adjusts from there. Is compounded hormone therapy ever the right choice?

Occasionally, yes. All three medical groups leave room for it in narrow cases: a true allergy to an ingredient in the approved product, such as the peanut oil in some micronized progesterone capsules, or a dose that genuinely isn't made commercially. What they agree on is that preference or marketing alone isn't a good enough reason.

If compounding is suggested to you, it's fair to ask why an approved product won't work in your case. A good clinician will have a specific answer. "It's more natural" or "it's customized to your saliva test" isn't one.

What should you ask your doctor about bioidentical hormones?

Bring the vocabulary, not a verdict. Ask whether what you're being offered is FDA-approved, which exact hormones and forms are in it, how the dose was chosen, and how the lining of your uterus will be protected if you still have one. Those four questions separate a careful plan from a sales pitch quickly.
