Paroxetine vs. Venlafaxine for Menopause Hot Flashes

The short answer

Low-dose paroxetine (Brisdelle, the only FDA-approved non-hormonal pill for hot flashes before 2023) and venlafaxine (off-label, with better sleep-specific trial data) are decades-old, cheap generics that still treat menopause hot flashes and insomnia. Paroxetine strongly blocks the liver enzyme that activates tamoxifen, making it risky for breast cancer survivors; venlafaxine doesn't carry that same risk in the same trial data.

I almost skipped writing about these two. They felt like old news next to Veozah and Lynkuet — the shiny, purpose-built drugs everyone's talking about now. But a reader on tamoxifen for breast cancer emailed asking why her oncologist had waved off paroxetine but seemed fine with venlafaxine for the exact same hot flashes. That's not a small question, and it's not one the newer drugs' marketing ever has to answer.

Paroxetine and venlafaxine are antidepressants first, repurposed for hot flashes decades ago, long before fezolinetant or elinzanetant existed. They're cheap, generic, and well-studied — and one of them carries a real, well-documented interaction that the other doesn't. That difference deserved its own article, not a footnote in someone else's.

The honest framing up front: both drugs have genuine trial evidence for hot flashes, and mild-to-moderate improvement in sleep tends to follow. The decision between them usually comes down to two things: whether you're also managing depression or anxiety, and whether tamoxifen is anywhere in your medication list.

What are paroxetine and venlafaxine, and how do they differ?

Paroxetine is an SSRI (selective serotonin reuptake inhibitor); venlafaxine is an SNRI (serotonin-norepinephrine reuptake inhibitor). Both were developed as antidepressants, and both were later found, sometimes by accident, to reduce hot flashes at doses lower than typically used for depression — paroxetine at 7.5–10mg, venlafaxine usually at 37.5–75mg extended-release.

The FDA-approved version of paroxetine for hot flashes is a specific 7.5mg capsule sold as Brisdelle. Venlafaxine has no FDA approval for hot flashes at all; doctors prescribe it off-label, which is common and legal, but means the manufacturer never ran the trials needed for that specific label. Both require a prescription, and neither is hormonal, so neither carries estrogen's blood-clot or breast-cancer considerations.

Do they actually help you sleep, or just quiet the hot flashes?

Venlafaxine has the more direct sleep evidence. A dedicated 2015 sub-analysis of the MsFLASH trial data measured insomnia and sleep quality specifically, not just hot flash counts, and found a real, statistically meaningful improvement. Paroxetine's trials tracked hot flash frequency as the primary outcome; sleep improvement is inferred from fewer night-time awakenings, not measured as its own endpoint.

That's not a knock on paroxetine's real-world usefulness — fewer 2 a.m. hot flashes genuinely means fewer 2 a.m. wake-ups for a lot of women. It's simply a gap in what's been formally studied, the same kind of distinction we found comparing Veozah and Lynkuet's sleep evidence last week.

What does the venlafaxine sleep evidence show specifically?

The clearest data comes from a 2015 Sleep journal analysis of the MsFLASH network's head-to-head trial: 339 women with hot flashes randomized to low-dose estradiol, venlafaxine XR 75mg, or placebo for 8 weeks, with insomnia and sleep quality tracked as dedicated outcomes, not an afterthought.

Venlafaxine performed roughly as well as low-dose hormone therapy on both measures — a genuinely useful finding for a woman who can't or won't take estrogen but still needs a real answer for fragmented sleep, not just a milder hot flash count.

What does the paroxetine (Brisdelle) evidence show?

Paroxetine's FDA approval in June 2013 rested on two placebo-controlled trials. The first, 606 women over 12 weeks, found a median hot flash frequency reduction of 5.9 per day with paroxetine versus 5.0 with placebo. The second, 568 women over 24 weeks, found 48% of users achieved a 50%+ reduction, versus 36% on placebo.

The effect held through six months, the longest point measured in either trial. Until fezolinetant's 2023 approval, Brisdelle was the only FDA-approved non-hormonal drug for hot flashes on the market — a distinction that mattered enormously to women with a history of hormone-sensitive cancer who'd previously had nothing but off-label options.

What's in the boxed warning, and should it worry you?

Both drugs carry the standard boxed warning every SSRI and SNRI has carried since 2007: a documented increased risk of suicidal thinking and behavior in patients under 25, identified in trials for depression and other psychiatric conditions, not in hot-flash trials specifically. The FDA requires the warning on the drug class, regardless of why it's being prescribed.

This warning is about a specific, younger population and a specific use. For a midlife woman without a personal history of depression, anxiety, or suicidality, it's a labeling requirement worth understanding, not on its own a reason to rule the drug out. Still, mention any mood history to your prescriber, and report any new or worsening mood changes promptly once you start.

Why does the tamoxifen interaction matter so much?

This is the difference that actually separates these two drugs, and it's the reason a reader's oncologist steered her away from paroxetine specifically. Paroxetine is one of the strongest inhibitors of CYP2D6, the liver enzyme that converts tamoxifen into endoxifen, its active, cancer-fighting form. Block that enzyme, and tamoxifen may not work as well as it's supposed to.

The FDA's own Brisdelle label reflects this: it names paroxetine as a strong CYP2D6 inhibitor and recommends considering avoiding it alongside tamoxifen. Not every later study found the same mortality signal, and the debate over how much this pharmacokinetic effect changes real-world outcomes hasn't fully settled.

But given a cheap, effective, well-studied alternative exists in the same drug class, most oncologists simply route around the question rather than argue it.

If you take tamoxifen, this is worth raising by name at your next appointment — not just "an antidepressant," but paroxetine specifically, alongside fluoxetine and (to a lesser degree) sertraline, all stronger CYP2D6 inhibitors. Venlafaxine, citalopram, and escitalopram are the options usually considered safer alongside tamoxifen.

None of these is universally "best." A woman with mood symptoms alongside hot flashes and no tamoxifen in the picture may do very well on either paroxetine or venlafaxine. A breast cancer survivor on tamoxifen has a much clearer path toward venlafaxine, gabapentin, or one of the newer non-hormonal drugs instead.

Who's a good candidate, and who should skip these?

These are worth discussing with a doctor if hot flashes and sleep disruption bother you, hormone therapy isn't right for you, and cost or coverage matters — both are inexpensive generics most plans cover easily. They're a logical first step if low mood or anxiety is part of the picture too, since that's what the drug class treats.

That's a real, practical advantage worth naming honestly, not just a consolation prize for being older.

Starting either means a conversation with your doctor about your full medication list — tamoxifen especially, but also other CYP2D6-dependent drugs and anything else that affects serotonin, to rule out interactions before the first dose.
